Once you book your appointment with our doctor, you will need to complete your Patient Intake Form located in your member area.  Your appoint will not be confirmed until after you have completed your Patient Intake Form.  

 

All appointments must be booked online and paid via credit or debit card.   We no longer allow bookings over the phone. Make sure you visit our Qualifying page before you book.  One of our staff members will be in touch after you book your appointment.

 

If you are an ARMMJ Industry Worker, then please email us a copy of your ARMMJ Industry Worker Card, and then call us before booking.
